Members of birth and adoptive families may contact the Department of Public Health and apply for non-identifying or identifying adoption information through the Illinois Adoption Registry and Medical Information Exchange. There is a small fee.

Confidential Intermediary Services of Illinois
The Illinois Adoption Act also provides a means by which birth relatives may seek confidential information. The confidential Intermediary Service of Illinois (CISI) has been created to assist interested birth relatives through CISI trained and certified Intermediaries. There is a standard fee (plus the Intermediary’s miscellaneous out-of-pocket expenses) for the service.
