COVID-19 Vaccine Documents

Consent Form subject to change with further guidance from IDPH/CDC, last updated on 1/28/2021.

The Woodford County Health Department has begun vaccinating residents of Woodford County, plus those who live or work in Woodford, Tazewell, or Peoria counties as part of a tri-county effort.

Vaccines are given by appointment only. Appointments may be cancelled due to a delay in receiving vaccine.

Due to a very high volume of calls and limited staffing resources, WCHD may not be able to return your call if you have previously left a message. Continue to watch this website and our Facebook page for more information on ways to sign up for a vaccination appointment as it becomes available. Your patience is appreciated.

If you have a vaccine appointment with WCHD, you must print your confirmation email with barcode or be able to show barcode on your device. We ask that you wear short sleeves to the clinic to aid in the vaccination process. A face mask is required.  You will need to wait at least 15 minutes after receiving your vaccine for monitoring.

If you have already received your first dose, watch for an email with link to schedule your second dose or follow directions that were provided on half-sheet of colored paper when you checked out after receiving first dose.

If you received your first vaccination dose at with another provider or clinic, WCHD cannot schedule you for your second vaccination. You will need to return to the provider that gave you your first dose for your second dose of COVID-19 Vaccine.
