Woodford County Health Department to Host Back-To-School Immunization Clinic 

 

The Woodford County Health (WCHD) would like to remind the public that Illinois State law requires certain immunizations for children and adults enrolled in child care, school, or college.


WCHD will be hosting an after-hours, walk-in clinic on Wednesday, August 2, 2017 from 1:00-6:00 pm. Additionally, immunization appointments can be scheduled during regular business hours.


“Immunizations are one of our greatest defenses against many serious illnesses, the required vaccine schedule can often times be confusing and we are here to help,” says Melissa Theleman, Director of Nursing. “We recommend that parents get their children vaccinated according to the Centers for Disease Control and Prevention (CDC) and American Academy of Pediatrics (AAP) guidelines.”

 

To assist parents and caregivers with school immunization requirements, WCHD offers vaccinations for families with private insurance and the medical card (Title XXI), insurance will be verified the day of service. Self-pay services are also available. If you are unsure of your insurance eligibility, please call the office at 309-467-3064.


To receive immunizations at WCHD, residents must provide a copy of their immunization records and active insurance card. If a resident is under 18 years of age, a parent/legal guardian must sign required paperwork and be present during the immunization. Parents are encouraged to visit www.woodfordhealth.org for required immunization forms and bring the completed forms to the clinic.
