Passport Fees

What is needed when applying for a passport.

  • Original certified birth certificate
  • Driver's License/State Issued Identification
  • Any certified document if there has been a name change i.e. Marriage Certificate, Adoption
  • Passport photo from a vendor that provides said service. i.e. CVS, Walgreens, etc.
  • 2 checks are needed: one is for the application fee and a separate one is for the processing fee
  • If the applicant is under 16 years of age BOTH parents will need to be present to sign
  • If your passport is expired you will need to surrender the expired passport-If lost/stolen the proper form needs to be submitted with the application
