Before scheduling your vaccine, please read through the following information carefully to make sure that your appointment is scheduled successfully.

Unless you receive a confirmation number via email or text message, your appointment time is not valid.


IMPORTANT-As of May 13, 2021 residents age 12 and up are eligible to receive the Pfizer vaccine. Woodford County Health DepartmenDone Editingt does not regularly offer the Pfizer vaccine at this time (check clinic dates below to see if any Pfizer clinics are scheduled). WCHD has partnered with other agencies to offer this vaccine to 12-17 year-old residents. Please check with Peoria City/County Health Department, or Tazewell County Health Department for Pfizer clinics or area pharmacies. Information and appointment times can be found on those agency websites or social media pages.

If you would like to pre-register at any of the walk-in clinics, please click on the date you would like to attend. If you do pre-register, bring the confirmation email/bar code that you received. If you do not pre-register, be sure to bring an ID.


  • Wednesday, August 4, 2021  9:00 AM - 11:00 AM
    • Pfizer, Moderna and J&J will be offered at this clinic
    • If you are receiving 2nd dose of Moderna, 1st dose must be on or before July 7, 2021
    • If you are receiving 2nd dose of Pfizer, 1st dose must be on or before July 14, 2021
  • Thursday, August 5, 2021  1:00 PM to 3:00 PM
    • Pfizer, Moderna and J&J will be offered at this clinic
    • If you are receiving 2nd dose of Moderna, 1st dose must be on or before July 8, 2021
    • If you are receiving 2nd dose of Pfizer, 1st dose must be on or before July 15, 2021
  • Friday, August 6, 2021 9:00 AM to 11:00 AM
    • Pfizer, Moderna and J&J will be offered at this clinic
    • If you are receiving 2nd dose of Moderna, 1st dose must be on or before July 9, 2021
    • If you are receiving 2nd dose of Pfizer, 1st dose must be on or before July 16, 2021
  • Tuesday, August 17, 2021 9:00 AM - 11:00 AM
    • Pfizer, Moderna and J&J will be offered at this clinic
    • If you are receiving 2nd dose of Moderna, 1st dose must be on or before July 20, 2021
    • If you are receiving 2nd dose of Pfizer, 1st dose must be on or before July 27, 2021

You must bring your vaccine record card with you to have second dose information added. Make sure you print or have access to your appointment ticket/bar code if you preregister.

Please wear short sleeves to the clinic to aid in the vaccination process. A face mask is required. You will need to wait at least 15 minutes after receiving your vaccine for monitoring.
